Open settlements with a payment tier that needs no proof

Updated · Sources

Each settlement below has at least one payment tier that, according to its own claim form or court-approved notice, needs no receipts or other documents when you file. "No proof" does not mean "no rules": you still have to be a class member, file the claim form by the deadline, and sign it, often under penalty of perjury.

How to read this list

A settlement is listed only when we read the no-proof wording in its official claim form, notice or website, and only while its claim deadline is still ahead. The table sorts by deadline and, with JavaScript on, counts the days left from today in your browser. The website shown under each name is the one its court-approved documents give; this site files no claims and links to no claim-filing service.

The tiers are not alike. The CVS and Dr. Squatch documents state an amount, per claim or per product, that can still shrink if many people claim, and LSC's cash payment has a ceiling and is paid pro rata. Equifax and Skin360 split a fund equally among valid claims, and the ATM settlement divides its fund in proportion to the number of qualifying fees claimed. Several forms also say the administrator may ask for documents later. Our payments guide explains why pro rata amounts change.

What the no-proof tier is in each settlement

CVS digital privacy

Cash Payment B is up to $5 Court-authorized long-form noticechecked October 1, 2026 with no proof or explanation, and the amount may decrease if many people claim. Its exclusion deadline comes before the claim deadline. The rest of the facts are on our CVS digital privacy settlement page.

Non-bank ATM surcharges (Visa and Mastercard)

The claim form asks for your estimated number of surcharged withdrawals and says: “You do not need to provide any documentation at this time. However, the Claims Administrator may ask you for additional documentation or proof that supports your claim” Claim form on the official settlement websitechecked October 1, 2026. The form is signed under penalty of perjury. Deadlines, payment methods and timing are on our ATM fee settlement page.

Equifax credit-score error

This case, In re Equifax Fair Credit Reporting Act Litigation, No. 1:22-cv-3072-LMM-CCB in the U.S. District Court for the Northern District of Georgia, is about a coding issue at Equifax that changed some credit scores or credit attributes that Equifax reported to third parties from March 17, 2022 to April 8, 2022. Who is in the class is decided by Equifax's own analysis; the notice says: “If you received an email or mail notice, then according to Equifax’s records, you fall within this group.” U.S. District Court, N.D. Ga.checked October 1, 2026

There is one tier. Every valid claim gets “an equal (pro rata) share of the Net Distribution Amount,” and the claim form asks for your Claim ID, name, address, email address, payment method and signature; no documents are asked for Settlement administrator for the court-approved settlementchecked October 1, 2026. The notice gives the plaintiffs' own estimate of $95 to $280 per claim and says the real amount may be higher or lower; it is their estimate, not ours. On payment, the notice says: “If you would like an electronic payment, you must electronically file the online claim form.” The exclusion deadline is November 27, 2026, postmarked, and the final approval hearing is set for January 22, 2027.

The notice and claim form call the administrator only "Settlement Administrator", give the address Equifax FCRA Litigation, P.O. Box 301132, Los Angeles, CA 90030-1132, and ask readers not to contact Equifax or the court. We could not open the official website, so this page names no administrator company, and there is no separate Equifax page on this site.

LSC data breach (Planned Parenthood lab tests)

The notice is for people employed by LSC, or who received or paid for lab tests ordered by a Planned Parenthood health center on or before October 27, 2024. Every class member may claim a cash payment of up to $1,000, pro rata Court-authorized long-form notice, linked on the official sitechecked October 1, 2026, with no documents. Every class member may also claim two years of Medical Shield Complete by CyEx, again with no documents. A separate payment for out-of-pocket losses, up to $5,000 with documents, does need proof. Online claims are due November 24, 2026, and a mailed form must be postmarked by the same date. The deadline to opt out or object, October 26, 2026, comes first.

Skin360 (Illinois)

The class is people who, while in Illinois, did a Skin360 skin assessment between December 9, 2019 and May 5, 2023. Each valid claim gets an equal pro rata share of the net fund, an amount not known yet Claim form on the official settlement websitechecked October 1, 2026. The claim needs no documents with the claim, though the administrator may ask for them (identity or proof of being in Illinois). The claim form asks for the dates you used Skin360 and, if you no longer live in Illinois, the dates you were there. The notice adds: "Any checks issued to Settlement Class Members will expire and become void 90 days after they are issued, and no replacement check will be issued for expired checks." The opt-out and objection deadline is November 10, 2026.

Dr. Squatch "natural" labeling

The class is anyone who bought Covered Products in the United States from November 1, 2018 through August 29, 2026. Covered products are products with "natural" on the label. Without proof of purchase you can claim up to 5 products Mail-in claim form on the official settlement websitechecked October 1, 2026 at $0.50 per product, attested on the form; claiming more products requires proof of purchase. The $0.50 per product may be adjusted pro rata. One claim form per household. The exclusion and objection deadline is October 13, 2026.

Why a settlement you know is not here

The Kroger prescription settlement lets claimants under a set amount skip documents when filing, but its notice says people not on Kroger's list of known buyers will be asked for documents, so it is on its own page and not on this list. Settlements whose official notice or claim form we could not read are left out, whatever other websites say about them. Being absent from this list says nothing about whether a notice you received is real; our notice check covers that.
